Why Choose Connecting Families for Supported Living?

The Hospital to Home and Complex Care Transition Program is designed to provide tailored, person-centred support for NDIS participants as they move from hospital or high-acuity settings back into the community or into Supported Independent Living.

At Connecting Families, we specialise in facilitating smooth discharges and complex transitions, ensuring participants and their families feel empowered, safe, and supported every step of the way. For those with high intensity and complex care needs, our experienced multidisciplinary team works closely with allied health professionals to deliver comprehensive planning, clinical oversight, robust reporting, and skilled, engaged support workers. This integrated approach ensures not only a seamless hospital discharge but also a consistently high standard of care throughout the transition to independence, giving participants the confidence and stability they need to thrive in their new environment.

Our experienced and qualified clinical team works hand-in-hand with allied health professionals to ensure that the care delivered in Supported Independent Living aligns seamlessly with each participant’s individual care and therapy plans. From the moment of transition, we provide ongoing oversight of health and wellbeing, closely monitoring medication, reviewing health charts, and working alongside the wider care team to quickly identify and respond to any changes in needs.

To give participants the best possible support, our clinical team also facilitates with the allied health cares tailored training sessions, ensuring they are confident, skilled, and equipped to meet specific care requirements. This not only helps participants feel safe and supported but also gives families peace of mind knowing that their loved one is cared for by a team that truly understands their needs.

In addition, our team assists with documentation and reporting for NDIS plan reviews, clearly communicating progress, achievements, and ongoing needs. This holistic and collaborative approach ensures care is consistent, transparent, and always focused on delivering meaningful outcomes. At Connecting Families, clients can trust they are supported by a dedicated team committed to safety, quality, and empowering independence

At Connecting Families, we believe true care goes beyond the initial transition — it’s about providing consistent, reliable, and evolving support that adapts as each client’s needs and goals change. That’s why every participant is surrounded by a dedicated care team, including Client Services, Clinical, and a Team Leader, who work together to provide a wraparound approach to care.

This team doesn’t just check in occasionally; they are actively engaged with the client’s wider stakeholder group, including family, allied health professionals, and other supports. Regular meetings and reviews allow us to celebrate achievements, monitor progress towards NDIS goals, and make timely adjustments to care or therapy plans. This collaborative approach means clients receive seamless, person-centred care that grows with them.

Our ongoing support model also gives families and participants peace of mind. They know there is a consistent point of contact and a team that truly understands their unique circumstances, strengths, and challenges. By proactively identifying changes in needs and ensuring care teams are trained and confident in delivering specialised support, we provide stability, safety, and trust — the foundations for independence and wellbeing.

Choosing Connecting Families means choosing a partner who doesn’t just deliver services, but one who walks alongside clients and their families every step of the way, ensuring no one feels alone in their journey.

At Connecting Families, we know that clear communication and accurate reporting are essential to achieving the best outcomes for our clients. That’s why we provide comprehensive reports designed to support participants, their allied health team, and NDIS plan reviews.

Our reports go beyond the basics, bringing together shift notes, weekly progress updates, and health monitoring charts to give a complete picture of each client’s care and progress. This ensures allied health professionals and families have the information they need to make informed decisions, while also providing the NDIS with clear evidence of outcomes, achievements, and ongoing needs.

By combining day-to-day insights with structured reporting, we create a transparent and reliable record of care that helps clients stay on track with their goals, supports clinical oversight, and strengthens future planning. With Connecting Families, you can trust that nothing is overlooked — every detail is captured, communicated, and aligned to deliver the highest standard of support.

When I first came to Connecting Families Ally would spend most the day in her room. She often had what she called bad days where she would not even feel like cooking for herself. She would keep microwave meals in the freezer for when she was too depressed to cook and buy more every week as she used them.

I spent quite a bit of time getting to know Ally over the first few weeks and learned that she really enjoyed cooking and planning to cook. I spoke with Ally about changing how we shop and cook each week. We set aside a day where Ally and the other residents would sit down and plan out a menu for the week. Then Ally with the assistance of staff would plan out the shopping list. Ally slowly became in charge of the menu and cooking the dinners in the house.

As time past Ally had more and more of reason to come out of her room. She interacted with staff more often, even critiquing staff’s cooking. I believe it gave her a purpose and a feeling that she contributed to the house. Behaviours of concern dropped off over time and it has been months since she has bought a microwave meal.

We recently began supporting Tommy at our new Gold Coast house. Tommy had previously been under care of two other NDIS providers over the last 12 months. Both of these placements ended with Tommy in the hospital for a long period of time. Neither of those placements was pleasurable to Tommy or his brother Terry.

When I first met Terry I could see how dedicated he was to his bother’s care. However, after the last two placements he did not expect this one would end up much different, but he was willing to give us a chance. Thankfully the whole team worked together to give us the best shot of making this placement a success.

After the holidays met with Terry to see how he felt about the care his bother was receiving. Terry let me know that Connecting Families had exceeded all of his expectations. Tommy is well looked after and he feels he can take a step back. Terry trusted us enough that he took his first holiday in years last week.
